In Dragon’s Dogma 2, there are more than a few pretty rare mythological beasts, and in true developer form, you’re encouraged to go after them with a few applicable achievements.
While the Headless Horseman’s achievement (Before Dawn Breaks) is pretty straightforward, Dragon’s Dogma’s 2 Medusa’s achievements are a little more involved. There are three in all, all of which are easily doable, but will require a know how and tactics.
First things first, where is Medusa? Well, Medusa’s location in Dragon’s Dogma 2 is actually potentially missable and there’s only one spawn. Medusa can be found in a suspicious cave in the far southwest of Battahl - on the road to the Dragonsbreath Tower.
On the west side of the Nera’Battahl Windrift area in southwest Battahl, you’ll find a cave called the Caliginous Depths - you’ll know you’re in the right place because two logs will form a cross across the entrance and there’ll be stone figures littering the path outside of it.
How to decapitate Medusa and get a Preserved Medusa Head
First things first, you’re going to need to do a little prep to get this easily, although it’s actually possible to get a Preserved Medusa Head without the following. However, the below just makes it a whole lot easier.
You’re going to need to:
Change your vocation to Thief. This is because the Thief deals out a lot of hits in a short amount of time.
Have a Pawn with Lightning Affinity in your party, as they’ll buff you and you’ll do more damage when attacking Medusa.
Have 2 Ferrystones and a Portcrystal.
And finally, this one is not necessary, but will help: have stamina buffing rings equipped, allowing you to grab onto the Medusa for longer and do more hits. The Ring of Momentum is perfect for this, which will moderately boost your maximum stamina (you’ll get this as a reward for completing ThePhantom Oxcartside quest).
Once you have all that, head to the Caliginous Depths.
When you’re in there you want to make a beeline for Medusa, avoiding her petrification ray if that’s the first move she does.
You’re then going to want to grab onto her, and climb up to her head. And then, just spam “Carve” - which is the standard Thief button attack.
If she flings you off - which she totally will - then just rinse and repeat. Just make sure you’re hearing the critical whoosh every time you make a hit - which you’ll get if you attack her head.
Sometimes you’ll do enough damage to her and this will stun her. At this point, stay grappled onto her and keep spamming Carve - again, landing those weak spot hits. In no time whatsoever, you’ll insta kill Medusa and her Preserved Medusa Head will fly to the floor. But you can still get the Preserved Medusa Head without the stun.
Now, time is of the essence here. Grab the head and leave the cave. When outside, drop a Portcrystal if you have one and then use a Ferrystone to transport back to any of the cities or outposts - it doesn’t matter which, so long as there’s a storage container there.
All you need to do now is to store the Preserved Medusa Head, as it won’t decay in storage. And then we wait.
How to turn Medusa to stone
In about 7 days time, Medusa will respawn - you can sleep 7 days if you want to accelerate time or just do other stuff for 7 days, your call. When enough time has passed, and you’re ready to petrify Medusa, grab he Preserved Medusa Head out of your storage, head outside and use a Ferrystone to get back to Medusa (to the Portcrystal you put outside her lair).
When you’re back at the Caliginous Depths, head inside. When you’re inside, go to your item menu and equip the Preserved Medusa head (make sure you’re not moving when you try this, as otherwise the options will be greyed out).
Equipping the Preserved Medusa Head won’t equip it in the weapon slot, it’ll equip it in the Lantern slot. So, you have to use it like a Lantern (using and on the d-pad). This will “Brandish” the head, and all you need to do then is focus the red beam onto Medusa herself. After a good 20-seconds, Medusa will turn into stone, and the third and final achievement, “An Eye for an Eye” will now be all yours.
Now, if you have another Ferrystone, you can grab the Portcrystal and use it to get back to town to put it back in storage (it’ll likely no longer be preserved now, and will be normal), which will allow you to use it in the other side quest, “A Case of Sculptor's Block” - Fulvio has asked you petrify the dragon if at all possible.
Not only is that now possible, but you’ll also have all 3 Medusa achievements as a result too. You’ll also get the Medusan Spellbow the first time you defeat her as well.

Not bad for not a lot of effort. Congrats!
